First most: "Medieval 2: Total War" at 530 hours.
Second most: "The Elder Scrolls V Skyrim" at 116 hours.

Highest Recorded times:
Saint's Row: The Third - 133 hours
Assassin's Creed Brotherhood - 66 hours.
Actual playtime for these 2 is much less.
Saint's Row: The Third - 30 hours.
Assassin's Creed Brotherhood - 10 hours.
Edit: Fallout 3 has at least 80 hours in it of unrecorded playtime for the steam version due to me playing offline mode on my desktop, and at least 100 more hours for nonsteam.

Bah, I can't use my almost 1 year of /played in WoW? Fine...
1. Red Orchestra 2
2. Empire Total War
Rome 2 follows closely in the games I play the most...but those 2 are the ones I've spent the most time on.
